Certification Consultation and Contract Stage
01 Consultation
Inquiry and consultation for the general matter related to procedure and
method of promoting the certification.

Telephone, Mail or E- Mai and Meeting
02 Requesting Proposal
Download and fill the relevant standard questionnaire for certification, and submit it to CreBiz.
Method of Consultation :
Applying on the Web, FAX, Mail, E-Mail
03 Certification Proposal
The audit proposal that including the information relevant to Certification Scope, Audit M/D and its fee on the basis of the submitted questionnaire will be sent to the client by FAX or E-Mail

01 Audit Plan
After making the certification agreement, CreBizOM will inform to the client about audit team and its plan.
02 Stage 1 Audit
The purpose of stage 1 audit is verification of compliance with the relevant requirement for documentation.
03 Stage 2 Audit
The stage 2 audit will be conducted after completing the stage 1, audit for the verification of md effectiveness of the system.

The nonconformities that detected by conducting the audit shall do corrective action and submit the NCR to CreBizQM. |
Review the submitted document and verify their validation. if needed, the On-Site verification audit can be conducted on the basis of the validation result |
The approval of registration for the certificate will be determined by the verification review, and the verification review will be conducted for the client that complete the certification audit its corrective action. |
The certificate will be issued after the approval of registration. |

Post-stage of initial certification
01 Surveillance Audit
The purpose of the periodic surveillance audit is the system retention and continual improvement for the certified organization. The organization shall take the surveillance audit with the determined interval.
02 02. Recertification Audit
Recertification audit will be conducted in accordance with the similar procedure on every 3 years from the registration of initial certification, the certificate will be re-issued when the organization meets the requirement of the standard
